Substance Use–Related Information
Information related to substance use, substance use disorder treatment, or referrals for such treatment is subject to heightened privacy protections under federal law.
- This information will not be disclosed without your specific authorization unless permitted or required by law.
- Disclosure for law enforcement, legal proceedings, or non-treatment purposes is strictly limited.
- We do not use or disclose substance use information to discriminate against you in employment, housing, insurance, or access to care.
Women’s Reproductive Health Information
Information related to reproductive health care, including but not limited to:
- Pregnancy-related care
- Miscarriage management
- Contraception
- Fertility and reproductive planning
- Reproductive health outcomes
is afforded additional federal privacy protections.
- We will not disclose reproductive health information for the purpose of investigating or imposing liability on a patient or provider for legally obtained reproductive healthcare.
- This information will not be released without proper authorization unless required by law for treatment, payment, or healthcare operations.
- We will not cooperate with requests that are intended to identify, investigate, or penalize individuals for receiving lawful reproductive healthcare.
OTHER PERMITTED USES AND DISCLOSURES
We may use or disclose your PHI without authorization when required by law, including:
- Public health reporting
- Health oversight activities
- Workers’ compensation claims
- Preventing serious threats to health or safety
All disclosures are limited to the minimum necessary.
USES AND DISCLOSURES REQUIRING YOUR AUTHORIZATION
We will not use or disclose your PHI without your written authorization for:
- Marketing purposes
- Sale of your health information
- Disclosure of psychotherapy notes (if applicable)
- Certain disclosures of substance use–related information
You may revoke an authorization at any time in writing.
YOUR RIGHTS REGARDING YOUR HEALTH INFORMATION
You have the right to:
- Access your medical records
- Request corrections to your records
- Request restrictions on certain uses or disclosures
- Request confidential communications
- Receive an accounting of disclosures
- Obtain a paper copy of this Notice at any time
OUR RESPONSIBILITIES
- We will notify you if a breach occurs that compromises your PHI
- We will not retaliate against you for exercising your privacy rights
- We will comply with all applicable federal and state privacy laws
